Mental Health Coordinator

The Mental Health Coordinator manages daily clinical operations and patient flow to ensure the Mental Health Program meets CCBHC access-to-care standards. This role supervises full-time behavioral health clinicians and independent contractors, coordinates timely appointment scheduling and patient assignments, and works with the care coordination team to support whole-person integrated care. The Coordinator collaborates closely with the Quality Assurance and Compliance Department to monitor operational metrics and support regulatory adherence. This position is essential to delivering efficient, high-quality, culturally responsive services aligned with MCMH's CCBHC mission.

Qualifications

  • Master's degree in Social Work, Mental HealthCounseling, Psychology, or a related behavioral health field.
  • Current clinical licensure in New York (LCSW,LMHC, LMSW, or equivalent).
  • Minimum two years of full-time experience inoutpatient mental health, substance use, or integrated care settings.
  • Strong understanding of CCBHC standards and acommitment to trauma-informed and culturally responsive care.
  • Demonstrated ability to manage staff performanceand support professional development.
  • Excellent verbal, written, and organizational skills.
  • Ability to multitask, prioritize, and thrive ina fast-paced team environment.
  • Bilingual proficiency in English and Spanish.
  • Experience with electronic health record systems and data analysis.
  • Experience managing staff members.

Responsibilities

Access to Care and Program Operations

  • Ensure the program meets or exceeds CCBHC access-to-care requirements, including same-day or next-day scheduling for new patients
  • Monitor appointment capacity across the mental health team to prevent scheduling bottlenecks
  • Manage caseload assignments to optimize both clinician capacity and patient outcomes to maintain service accessibility
  • Track key performance indicators related to timeliness and continuity of care
  • Review scheduling data regularly and make real-time adjustments to maintain access standards

Staff Management and Development

  • Directly supervise full-time behavioral health clinicians and independent contractors
  • Conduct performance monitoring and provide feedback to clinical staff
  • Facilitate professional development and training opportunities
  • Support onboarding of new staff and orient them to agency workflows
  • Serve as the primary operational point of contact for clinical staff and resolve workflow issues that impact service delivery

Patient Assignment and Continuity

  • Manage patient assignments from the intake to therapy within the Electronic Health Record system
  • Conduct extended evaluations to deliver short-term clinical support to high-risk patients
  • Ensure timely transitions that support continuity of care
  • Coordinate with the Access and Intake Department to streamline patient flow throughout the system

Integrated Care Coordination

  • Serve as the primary operational liaison between the Mental Health Program and the substance use team, care managers, peer specialists, nursing, and psychiatry.
  • Facilitate collaboration across programs to identify and address the needs of patients with complex presentations
  • Ensure coordination of services and reduce fragmentation of care
  • Provide translation services to support equitable access for diverse patient populations

Data, Quality and Regulatory Support

  • Use the Electronic Health Record and other data systems to track clinical caseloads and monitor operational metrics
  • Identify opportunities for service improvementbased on data analysis
  • Prepare monthly reports on clinical staffing,caseload distribution, and access-to-care metrics for the Director of MentalHealth
  • Collaborate with the Quality Assurance andCompliance Department to support adherence to HIPAA and regulatory standards
