Peer Support Specialist

MCMH seeks a Certified Peer Specialist (CPS) to support Mental Health services. Peer Support Specialist have experienced either behavioral health/substance use concerns themselves and/or have supported family members through their experiences and have familiarity in navigating the behavioral health system. Peer Support Specialists use their experiences to support and empower others in their recovery and help people improve their overall well-being. The Peer Support Specialist will provide services onsite, in the community, and patients’ homes’ as necessary.

Qualifications

  • NYS Certification, or provisional NYS certification with commitment to satisfy
  • High School Diploma or equivalent
  • Bilingual Spanish, preferred

Responsibilities

  • Assist and guide mental health adults and older adult patients toward identifying and achieving specific goals defined by the patient and specified in the individual treatment plans by providing individualized peer support, wellness counseling, and peer-run support groups as appropriate.
  • Provide evidence-informed support to MCMH patients.
  • Provide outreach to the community at large to locate, intake, and link recipients to care to help facilitate and develop strong connections with patients and families in need of services and support.
  • Collaborate with our partnering agencies to ensure a smooth transition with all referrals, including responding to emails from referral sources and presenting available services at partner sites.
  • Accurately document services and contacts in the electronic health record in a timely fashion.
  • Attend weekly individual and group supervision sessions.
  • Perform other duties as assigned by supervisor.
